Healing Hearts Medical has been awarded a Texas Medicaid waiver contract to provide Adult Mental Health (HCBS-AMH) supportive housing and psychosocial services for adults diagnosed with Serious Mental Illness (SMI).

How It Works
Your Path to Structured Recovery
Referral or Inquiry
Families, hospitals, case managers, or individuals contact Healing Hearts Medical directly by phone, email, or referral form.
Eligibility Review
We verify Medicaid eligibility, HCBS-AMH requirements, and coordinate directly with discharge planning teams.
Housing Placement
Qualified individuals are placed in one of our Adult Mental Health supportive living homes within 24–72 hours.
Recovery Support
Residents receive structured daily support including medication assistance, life skills, and peer support services.
